Enter the date you were born, like this: mm/dd/yy
For example:

If you were born October 1, 1997, enter  10/1/97  or  10/01/97
If you were born July 4, 1969, enter  7/4/69  or  07/04/69

Note: Most common U.S. date formats will work. Say your birthday is March 17, 1994. Any of the following will work: 3/17/1994, 3/17/94, 3-17-94, 03-17-94, 031794, 03171994, 3.17.94, 03 17 94 and others. Spelling out the month (like 'March 17, 1994') will NOT work.
